Other problems noted included soda fountain nozzles that had a heavy mold/residue build up and a school kitchen that has insufficient hot water to clean the area.

The Pennsylvania Department of Agriculture keeps records of food safety inspections at restaurants and food vendors.

Berks County is divided into three jurisdictions: Reading, Muhlenberg Township and the rest of the county. The results will be posted every other week.



If a food provider is labeled "out of compliance," that means it has one or more violations that require a return visit by an inspector. The establishment is given time to fix the problem and remains open for business.

Below are the results of inspections conducted between April 15 and April 30, 2019, and include the inspector's comments on violations.

Gourmand LLC, 706 Brownsville Road, Sinking Spring , April 29, one violation. Food Facility Person in Charge not able to produce records showing that raw or partially cooked ready to eat fish has been frozen by the facility or approved supplier as required, before use or sale.

Little Caesars, 3580 Penn Ave., Sinking Spring April 29, five violations. Observed pizza oven equipment, in kitchen area, with an accumulation of dust, dirt, food residue, debris on the rollers of the pizza oven a non-food contact surfaces. Pan rack in front of dough table has a food residue/grease build up on shelving of rack and leg areas. Damaged ceiling tile above bain marie still has to be replaced. Plumbing system not maintained in good repair - observed drain line leaking at the warewashing sink. Ceiling vents in kitchen area were cleaned but tiles and light covers around vents were not all cleaned and are dirty, dusty, and in need of cleaning.

Pizza Hut #028202, 1192 W Penn Ave , Womelsdorf April 29, six violations. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in pizza prep equipment. Handwash sink in back kitchen area is clogged and must be repaired. Rear door located in the storage room area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. Kitchen ceiling vent, ceiling tiles at vents and light cover area of the food facility is extremely dirty, dusty, and in need of cleaning. Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. Corrected. Observed fryer cooking equipment sides, floor area under fryer and table next to fryer in the kitchen area, with encrusted grease and soil accumulation. Observed racks proof box area, with an accumulation of dirt, food residue, debris on bottom of rack and wheel areas a non-food contact surfaces.

Rocky Family Restaurant, 4055 Conrad Weiser Parkway, Womelsdorf April 29, 25 violations. "Cigarettes, cigarette butts, found on the dry storage room area on floor next to ice machine, during the inspection. Pans of food observed in the kitchen walk in refrigerator area, slimy to the touch/spoiled/moldy and is adulterated. Corrected by disposal. Severely dented, swollen, distressed canned items observed in kitchen are and intended for use or sale in the food facility. Corrected. Open trays of frozen chicken croquettes in walk in freezer area, is an unpackaged food stored or displayed in direct contact with ice condensate dripping from fan box. Discarded. Walk in cooler has condensate build up on fan box dripping down onto food products and leaving stagnant water in bottom of walk in. Boxes of produce removed from that area. Old food containers of breading, stagnant water with floating butter and butter sitting out at side kitchen area. Discarded. Burger King #4750, Route 222 & Quarry Road, Kutztown, April 26, four violations. Floor grout is deteriorated in some areas and some coving tiles are damaged; compartment sink is not yet sealed at the junction with the wall (after replacing the FRP behind the sink); Some food contact surfaces in the kitchen currently have a coating of construction dust. All surfaces must be cleaned and sanitized before reopening; and Roof is leaking water in multiple places along the drive-thru/3-compartment sink wall.

Cielito Lindo Mexican Restaurant, 278 W. Main St., Kutztown, April 26, three violations. Thermometer for one of the front cold hold units is broken; wood pressing unit in the front area is not smooth, non-absorbent; and Ceiling tiles missing in the back area near the 3-compartment sink need to be replaced.

Holiday Inn, 6170 Morgantown Road, Morgantown, April 26, 14 violations. The Person in Charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection; Paper towel dispenser empty at the handwash sink in the bar area; Assorted cheeses held in refrigerated top drawer on cookline at 46-47 °F, rather than 41°F or below as required. COS by disposing of product; Dispensing unit for sanitizer, in 3 bay sink, was observed to have black filth observed in tubing and requires cleaning/ repair; Live (>50) ants observed on the premise of the food facility in the kitchen floor area. Observed sodas, milk, other drinks stored directly on the floor in the walk in cooler, rather than 6 inches off of the floor as required; Toilet rooms do not have a self-closing door; 4 Pints, One 1/2 Gallon, and 1 Gallon of skim milk and whole milk were observed for public sale with a April 22nd sell by date. Corrected onsite by disposal. Loose rubber door gaskets observed on the 3rd door of the walk in cooler.

Circle S Ranch House, 2738 Penn Ave., West Lawn , April 24, five violations. Observed deeply scored bain marie cutting boards not resurfaced or discarded as required. Floor / wall / ceiling in the walk in cooler area is made of painted matal and is cracked /peeling/ roughened / and is not a smooth, easily cleanable surface. Observed cardboard being utilized as a floor covering. Preset tableware is not wrapped, covered or inverted, or being removed at a frequency to assure tableware remains clean and sanitized. Microwave bracket and cord above the cook line steam table area of the food facility is dusty and in need of cleaning.

Go Fish Seafood @ Body Zone, 3103 Papermill Road, Wyomissing , April 24, seven violations. Soap was not available at either of the handwash sinks. Corrected. Working containers (spray bottles) of cleaner were stored hanging on a shelf with clean food utensils in the back prep area. Corrected. Main Street Pub & Restaurant, 505 Main St., Bally , April 24, four violations. Observed the fryer and grill equipment, in kitchen area, with an accumulation of dirt, grease, food residue underneath grill and floor area under, and wall behind the fryers. Observed the spray nozzle at the 3 bay sink, with an accumulation of dust, dirt, food residue, debris on non-food contact surfaces. Observed the 2 door True Freezer, and refrigerator with an accumulation of dirt,food residue debris on the non-food contact services. The latch to the door of the walk in cooler is broken. -Observed a plastic spray bottle at the bar, and one in the kitchen with no identification label. Corrected onsite. Ice machine plastic divider, a food contact surface, was observed to have a light pink slime mold build up and was not clean to sight and touch. Observed single-service, single-use articles stored in salad station shelf area which were not in the original protective package, covered or inverted. Corrected. Kitchen ceiling vents, surrounding ceiling tiles, wall at fans and light cover areas of the food facility are extremely dusty and in need of cleaning. Brooks Cafe LLC, 1139 W. Ben Franklin Highway Unit 2, Douglassville , April 22, two violations. The interior of the ice machine, a food contact surface, was observed to have residue and was not clean to sight and touch. Corrected. The scoop at the ice machine is being stored in the adjacent bread rack. This is not necessarily a clean surface.

Daniel Boone Area Schools Amity Intermediate , 200 Boone Drive , Douglassville , April 22, six violations. The food-contact surfaces of equipment and utensils did not reach 160 degrees F during the sanitizing cycle of the high temperature dishwasher as evidenced by this inspector's thermometer (calibrated at 32.1 degrees F during this inspection). The dial on the machine was indicating a rinse temperature of 180 - 190 degrees while running dishes through, however, this inspector's thermometer was run through with the dishes three times and had readings of only 135 - 143 degrees F at dish level. The hot water used for washing in the mechanical warewashing machine did not reach 160°F as is indicated for a minimum temperature by the plate on the machine. This dial did not indicate a temperature higher than 130 degrees F during this inspection. Food facility hot water heater is not producing enough hot water to supply the kitchen/cafeteria sinks at the time of this inspection. Employees stated that the water is briefly warmer in the morning when they arrive, but is never 'hot' and any warmth quickly disappears. They heat water in the cooking equipment to use for cleaning and sanitizing some surfaces. Soap was not available at two of the handwash sinks in the kitchen area. The sink closest to the office does not have any soap/soap dispenser, and the one near the 3-compartment sink has a soap dispenser that is located too far away to be convenient (approximately 3 feet away above the 3-compartment sink). There are no paper towels available at the handwash sink closest to the office in the kitchen area. Paper towel dispenser is located across the room and not at the handwash sink in the dishwash area. The handwash sinks located in the kitchen and cafeteria employee restroom area do not have water at a temperature of at least 100°F. The actual temperatures taken during this inspection ranged from 72 - 76 degrees F. Out of compliance.

McDonald's #27056, 1188 W. Ben Franklin Highway, Douglassville, April 22, seven violations. Coffee cup being used as an ice scoop at the front drive-thru window. Corrected. Wall in the back drive-thru window area has a hole/missing tile and is in need of repair. There are missing tiles and a hole in the wall at the mop sink. There is standing dirty water in the area where coving tiles are missing. Working container (spray bottle) in kitchen prep area, used for storing chemicals, cleaners taken from bulk supplies, was not marked with the common name of the chemical. Corrected. There is a significant accumulation of grease on the floor between the grills. Observed plastic food containers stored open and not inverted to protect the food contact surface in the kitchen area. Corrected. Thermometer for ensuring proper temperature of equipment is not available or readily accessible in the small Beverage-Air refrigerator next to the fryers. Dunkin Donuts, 1 Cheltenham Drive , Wyomissing , April 19, seven violations. Dipper well is not turned on and a scoop is being stored in standing dirty water. Corrected. Paper towel dispenser empty at the handwash sink in the front area. Corrected. Thermometers for ensuring proper temperature of equipment are not available or readily accessible in the small under-counter refrigerators at the front counter and near the ice machine. Observed an employee jacket and some cleaning items (erasers, blue cloth) stored on sandwich pouches in the back area. Corrected. Plumbing system not maintained in good repair - observed a significant amount of water leaking at the mop sink faucet. There is standing dirty water and other trash on the floor under the front counter (near the small refrigerator). Observed the ice bucket stored hanging on the front of the handwash sink subject to splash contaminating the food contact surface when employees wash their hands. Corrected.

Sonic Drive In (Exeter Commons), 4515 Perkiomen Ave., Reading , April 19, one violation. A food employee was observed assembling cheese burgers - a ready to eat food - with bare hands. Corrected.

Little Caesars, 3580 Penn Ave., Sinking Spring , April 18, seven violations. Bain marie has a water condensate build up in bottom and also has old spilled food product on bottom shelf and sides. eavy build up of old flour residue on flour bin at crust rolling machine. Ceiling tiles are damaged in the pizza bain marie area, and need replaced. The drain in the handwash sink in the front kitchen area is leaking. Pizza oven has excessive piles of burnt food debris under pizza conveyor catch trays. Hood vents have excessive dust build up. This is a repeat violation. Floors under racks and equipment have food debris and grease residue build up. Glass doors to walk in cooler have excessive food debris build up on handles of doors. Food Employees observed working with exposed foods with fingernail polish and / or artificial fingernails. There are no gloves for use in the facility. Rite Aid #11169, 23 N Elm St., Kutztown , April 18, one violation. There is a significant amount of standing (dirty) water in the bottom of the refrigeration unit used to store milk.

Brubaker Bargains, 1030 Church Road, Womelsdorf , April 17, three violations. Severely dented, swollen, distressed canned items observed in store and intended for sale in the food facility. Corrected however this is a repeat violation. Large sliding warehouse door located in the food storage room area of the food facility has large gaps at bottom where cement is chipped away and does not protect against the entry of insects, rodents, and other animals. Large warehouse door to the outside, located in the storage room area of the food facility is not self-closing and/or is being propped open. Corrected. Food facility does not have available chlorine sanitizer test strips or test kit to determine appropriate sanitizer concentration.

Miami Style Cafe, 7 Ridge Way , Womelsdorf , April 17, two violations. Food facility does not have available chlorine sanitizer test strips or test kit to determine appropriate sanitizer concentration. Working containers in sink area, used for storing *chemicals, cleaners* taken from bulk supplies, were not marked with the common name of the chemical. Corrected.

The Gathering Place Cafe, 8490 Lancaster Ave., Bethel , April 17, one violation. Toilet room in dining room does not have a self-closing door.

Turkey Hill Minit Market #124 , 3664 Seisholtzville Road, Hereford , April 17, two violations. Loose rubber door gaskets observed on several front doors of the walk in cooler. There is observable dirt/dust on the fan guards in the walk in cooler.

Observed various food stored directly on the floor in walk in bread cooler area, rather than 6 inches off of the floor as required. Walk in cooler has mold on coving tiles around floor areas and also some shelving units have dirt/mold build up on wire shelving. Ceiling condenser fan area and ceiling in walk in coolers of the food facility is extremely dusty and in need of cleaning.

Corrected upon inspection. The handwash sink in the prep area was blocked by pizza dough pans and was not accessible at all times for employee use.

The Person in Charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Removed immediately upon inspection. 3 rubber spatulas have cracks, and are not smooth and easily cleanable. Discarded immediately upon inspection. Dessert refrigerator (glass door)door tracks soiled. 1 cutting board with deep scores; discarded upon inspection.

Bojangles Famous Chicken N Biscuits, 3215 N. Fifth St., April 20,  two violations. Observed deeply scored cutting boards not resurfaced or discarded as required. Food employee observed storing clean food equipment, ice scoop holder, while wet, and not allowing time for draining and/or air-drying. Corrected immediately.

Food employees observed in food prep area (French Fries and Sandwich assembly), not wearing proper hair restraints, such as nets, hats, or beard covers. Observed tomato pieces in the handwashing sink, indicating use for purpose other than handwashing. Working containers of Peroxide Multi Surface Cleaner were stored above or on the same shelf with food, equipment, and/or single service articles on the prep tables adjacent to the ice machine.
